Fitness Connection Personal Trainer Contract
Welcome to Fitness Connection! We are excited to have you as one of our personal trainers. This contract outlines the expectations and responsibilities of both parties.
Personal Trainers
1. Personal trainers must be 18 years or older.
2. Personal trainers must have a current CPR certification.
3. Personal trainers must be certified in personal training through an accredited organization.
4. Personal trainers must complete a background check.
5. Personal trainers must adhere to the Fitness Connection code of conduct.
Clients
1. Clients must be 18 years or older.
2. Clients must have a current CPR certification.
3. Clients must be screened for contraindications to exercise.
4. Clients must sign a liability waiver.
5. Clients must adhere to the Fitness Connection code of conduct.
Responsibilities of the Personal Trainer
1. Personal trainers must create a safe and effective workout program for their clients.
2. Personal trainers must ensure that their clients are properly warmed up and cooled down.
3. Personal trainers must monitor their clients’ progress and adjust their programs as needed.
4. Personal trainers must provide their clients with educational materials about fitness and nutrition.
5. Personal trainers must be available to their clients during their scheduled sessions.
6. Personal trainers must maintain accurate client records.
Responsibilities of the Client
1. Clients must arrive on time for their scheduled sessions.
2. Clients must follow their personal trainer’s instructions.
3. Clients must wear appropriate clothing and shoes for their workouts.
4. Clients must not divulge their personal trainer’s confidential information.
5. Clients must adhere to the Fitness Connection code of conduct.
Fees
1. Fees are payable in advance.
2. Fees are non-refundable.
3. Fees may be prorated for missed sessions.
4. Fees may be increased with 30 days notice.
5. Fees may be decreased with 30 days notice.
6. Fees are subject to change at any time.
Duration of Agreement
1. This agreement is for a period of one year.
2. It may be terminated by either party with 30 days notice.
3. It may be terminated immediately for cause.
4. In the event of termination, fees are prorated for the remaining duration of the agreement.
5. The personal trainer is responsible for all outstanding fees.
This contract is an agreement between Fitness Connection and the personal trainer. By signing this contract, both parties agree to the terms and conditions outlined herein.

All Day Fitness Personal Training Studio
is a fitness and wellness center located in the heart of historic downtown Frederick, MD. Our team of certified personal trainers provides one-on-one and small group training, as well as nutrition counseling, to help you reach your fitness goals. Our studio is open Monday through Friday, 6am to 7pm, and Saturday, 8am to 2pm.
Our personal trainers are certified by the National Academy of Sports Medicine (NASM) and the American College of Sports Medicine (ACSM). We offer a variety of training options, including strength training, cardio conditioning, and Pilates. We also offer nutritional counseling to help you reach your fitness goals.
Our studio is located in the heart of historic downtown Frederick, MD. We are open Monday through Friday, 6am to 7pm, and Saturday, 8am to 2pm.
